Registration will be accepted in the front office:
 
The 2015-2016 Registration Packets are available for download below. If you are a returning student to any of the Bonsall Schools, you will only need the REGISTRATION PACKET. If you are NEW to the Bonsall School District, by NEW meaning you DID NOT attend one of the school listed above in the 2014-2015 School year, please complete both the NEW STUDENT packet AND REGISTRATION packet. 
 

 
Please have your packet completed and all documents ready for photocopy when you come to the office. This will make the process go faster. Thank you for your help!!!!
 

 
ALL NEW STUDENTS WILL NEED TO BRING TO REGISTRATION:
  • BIRTH CERTIFICATE
  • IMMUNIZATION RECORDS
  • 7th GRADE ONLY, Proof of the TdAP shot. A flyer is available below
  • 2 (TWO) PROOF OF RESIDENCY
ALL RETURNING STUDENTS WILL NEED TO BRING:
  • 7th Grade only TdAP Proof
  • 2 (TWO) PROOF OF RESIDENCY
These include: (we no longer accept Driver's Licenses)
  • Home Rental or Lease or Mortgage
  • Current Vehicle Registration
  • A form from State or Federal Government Agency
  • Current GAS Bill or ELECTRIC Bill
  • If you are a renter and DO NOT pay utilities because they are included in your rent or lease, we will need an original copy of the RENTAL/LEASE Agreement that utilities are included and a copy of a GAS or ELECTRIC Bill.  
 
Below you will find the attachments for the Library Young Reader, Library Rules, Activities Requirements, Physical Education Requirement  and TdAP for (7th Graders only.) These forms need to be printed and signed prior to Registration. This year each student has the opportunity to purchase a Lock from the PE Dept. for a locker along with the Sullivan Middle School PE Clothes, all three items equal $25.
 

 
SPORTS:
If you would like to participate in the SCHOOL SPORTS for 2015-2016, you will need the following forms before try-outs. Try-outs will be posted through the PE Dept. 
 
  • Proof of Insurance and Waiver Release
  • Athletic Participation Physical Form
  • Concussion
